If you are getting or will be getting long-term services and supports, you can meet in person with your Service Coordinator to go over what all of your needs are. This is called a “comprehensive needs assessment.”

Coronavirus Disease (COVID-19)

Visit the Centers for Disease Control and Prevention's (CDC's) website to learn more about the illness and how to help stop its spread. The CDC also offers information about coronavirus (COVID-19) in other languages and formats.

You can also visit the Pennsylvania Department of Health (DOH) website for information specific to Pennsylvania, including information on how to get tested for COVID-19.

Telemedicine/Telehealth

To help Participants get the best care possible, Keystone First CHC is currently covering telemedicine when offered by your doctor. Talk with your doctor about telemedicine options.

Refill of medicines

You may be able to get additional supply of certain maintenance medicines. Talk with your doctor or pharmacy. Also, ask your pharmacy about prescription delivery. Many pharmacies are now offering new delivery options due to COVID-19 concerns.

Dental care

At the direction of the DOH, many dentist offices have closed. This means that in most cases routine dental care is not being done right now.

If you need to see a dentist, please call the dentist you usually see. The dentist should be able to talk with you about how you can get care. 

If your dentist is closed, please be sure to listen to the entire message on the dentist’s voicemail. Most dentists are giving instructions on how you can reach them on the voicemail.

Are you a new Participant? Here is some important information (PDF) to get started with your health plan.

Thank you for choosing Keystone First Community HealthChoices for your health care needs. We are happy to have you as a Participant. If you cannot find the information you are looking for on the website or in the Participant handbook (PDF), you can:

  • Call Participant Services at 1-855-332-0729 (TTY 1-855-235-4976) to talk to a Participant Services representative, 24 hours a day, 7 days a week, or,
  • Call the Nurse Call Line at 1-855-332-0117.

Who we are

Keystone First Community HealthChoices (CHC) is a managed care organization providing health care under the Pennsylvania Community HealthChoices program. We offer health care benefits and services for Participants 21 years of age and older who have:

  • Medical Assistance (Medicaid) and Medicare,
  • Medical Assistance (Medicaid), Medicare, and long-term services and supports (LTSS), or
  • Medical Assistance (Medicaid) and LTSS.

Keystone First CHC works with our Participants to help them get the care they need, when they need it, and where they need it.

How to enroll

When you become eligible for Community HealthChoices, you will need to choose a Community HealthChoices health plan. The Department of Human Services (DHS) works with a company with enrollment specialists to help you choose a health plan.

The enrollment specialists can:

  • Help you enroll in a health plan.
  • Answer questions about your choice of health plans.
  • Help you if you decide to change health plans.
